Health care decisions are to be taken in a rationally justified way, into a pluralistic ethical arena and through an interdisciplinary dialogue among naturalistic and humanistic disciplines. A professional figure of clinical ethicist, working in specific departments, is needed to help people to deepen their personal moral evaluations, and to foster and facilitate coherent and shared decisions, both at individual, staff and institution level. Particularly at the bedside, paternalistic and directive counselling might be avoided, if some core competencies and skills in applied ethics are trained.
