In the light of a thorough review of some official documents by two important scientific societies, especially the SIAARTI (Italian Society of Anesthesia Reanimation and Intensive Care) and the SINPE (Italian Society of Parenteral and Enteral Nutrition), about artificial nutrition and hydration, the author tries to demonstrate how for scientific reasons, as well as from the ethical point of view, it is very difficult to incorporate these practices, regardless of whether they are considered ordinary care or medical therapies, into the Advance health care directives, given the great difference of the clinical picture and of the evolution of certain diseases. This leads to consider that it is pratically impossible to express this desire just because nobody can predict in what particular serious medical condition may possibly find herself/himself in the future. In fact, in terms of therapeutic, care and biological-instrumental monitoring there is a great difference between a patient in the ICU and a cancer patient (and if she/he is in terminal phase) compared to a person in a vegetative state or otherwise disabled. They are sick persons who need different "therapies", but the same care (cleaning, wounds cleansing, warming, mobilization, bowel control). As for artificial nutrition and hydration, the Author believes that they should always be offered, guaranteed and practiced, especially to severely disabled until they are effective and useful and don't take the shape of over treatment. From the perspective of bioethics, everybody accepts that nutrition hydratation and nutrition have a high human and symbolic value and considering whether to practice or to suspend them, it is claimed a strong decisionmaking power of the physician, who assesses on scientific parameters and never on a personal ethical judgment or conditioned by the evaluation of other people on the life quality of the patient.
